Agent Automatic Donation Form

The agent below agrees to deduct voluntary contributions from their commission check to the Coldwell Banker Realty Cares Foundation. These funds will be held by the foundation until it is deemed appropriate to donate. The donation will be granted to a public charity or other 501(c)(3) organization at the request of Coldwell Banker Realty Cares Foundation’s Advisory Committee.

To change your donation, please resubmit this form. To cancel your recurring donation please email your office inbox. They will contact licensing on your behalf.

  • I request that the following amount be deducted per side of a closed transaction from my commission check. (For example: If you are on the listing side and selling side of a transaction and request $25.00 be deducted,a total of $50.00 will be deducted. $25.00 per side.) Rental properties are also included in the program. Donations should be made in $5 increments.
    If you’d like to set a max deduction per year, please enter it in the Other field.
  • Deductions will begin within one week from submission date. The renewal is automatic each year. If you wish to stop this deduction, please complete the cancellation form. You will receive an annual contribution letter for tax purposes.
